If required, I will write to your referrer with a summary of your treatment and progress and will obtain your consent and also discuss with you the content of such reports before sending them.

If my professional opinion was that there was an immediate and serious risk that you might harm yourself and/or someone else or property, then I may have to share your personal information with your GP or the emergency services without first obtaining your consent, and in order to protect you or someone else. It is possible that your personal information may be requested by The Coroners office, Court of Lay or a Professional Body. In such circumstances you may have limited or no rights of refusal.
